You should be able to mux them together without splicing the streams.
At the end of disk 1 all streams will end and be "virtually replaced"
with the streams starting on disk 2. All in one big container file.
Mkvmerge supports appending files.
(I have yet to try how this works in practice, though. But that's one
way for tv services to share a common transmission service by using
dynamic PMT.)

I'm not aware of an automatic way to connect the parts, but you can
connect them manually by setting "File to Always Play Next:".
