Here are some useful pointers:

i) use the correct LIRC driver - you seem to be using the lirc_i2c
which is correct for the PVR-150 receiver
ii) make sure your LIRC devices are correct created in /dev (you may
need to install udev rules for this) and have usable file/user
permissions
iii) make sure lircd (the LIRC daemon) is starting using the correct
device - you may want a symlink between /dev/lirc and /dev/lirc0 (this
is the most common device problem I've seen)
iv) make sure you have the correct /etc/lircd.conf remote
configuration file in place (you'll want the Hauppauge one)
v) use LIRC logging and check your dmesg/syslog for messages relating
to IR - you'll want to pay attention to your ivtv and LIRC modprobe
output.

if there's anything else I can think of if none of this helps.
