[mythtv] multiple tuners, multiple frequency tables

Ok, I got it working so the frequency table is changed on the fly based on
the sourceid of the channel.  I've modified everything to make it work
right with an auto dbcheck update, except for the setup GUIs.  I just need
to remove the frequency table selection box from the global settings area
and add one to the video source setup screen.  When that's done (probably
tonight or this weekend) I'll send a patch.  I gather that right now there
aren't many (any at all?) people who need/want this but I think it makes
more sense to do it this way since it will be useful for us pcHDTV people
who also want analog cable.
