In the UK this is usually a red dot, I think this is an interactive TV service that was recorded along with the programme and perhaps whatever it linked to is no longer there. I have had much the same experience but with the red dot, interactive TV is called "red button" services here.

You can switch off interactive services in the frontend menu, so it's quite possible you have one frontend with it on and one with it off.

Apparently it's in:

Utilities/Setup -> Setup ->TV-Settings->Playback Onscreen Display.

Once in there, simply clear the tick/check in the the ‘Enable Interactive TV’ setting.
