Thanks - looks like with the same versions of everything your machine
is capable of picking up all the extra speakers and transfer protocols
whereas mine's only getting LPCM/Stereo - either my TV's not saying it
can support it, or there's a problem with my card like Jean-Yves
mentioned.

I just rebooted my machine (combined FE/BE) and disabled the onboard
audio - now myth actually picks up everything automatically and I'm
using "ALSA:hdmi:CARD=NVidia". plughw:0,3 and 0,7 also work.

Is there any way to tell which is which?

I have an Asus BRAVO 220 SILENT/DI/1GD2:
01:00.0 VGA compatible controller: nVidia Corporation GT216 [GeForce
GT 220] (rev a2)
01:00.1 Audio device: nVidia Corporation High Definition Audio
Controller (rev a1)

According to the wiki someone's managed to get the same (or a very
similarly-named card) working with multichannel - so is it just my TV
refusing to accept a multichannel stream?
