Unless Ben has 'ls' aliased to 'ls -a' you wouldn't see it with a
simple 'ls', as it hides dot files by default.

Still a possibility though for the problems described, either a
completely missing htaccess file, or more likely one containing
incorrect database connection information in the <Files mythweb.*>
section.
