I have it working, but it took some strange turns. It goes like:

-Started installation from scratch, picked defaults on most selections.
-Note: the IR selection screen only offered selections for the remote
control...nothing for IR blaster.
-Finished installation > rebooted > MCE remote control worked
-Exit MythTv and navigate to Mythbuntu control panel, then the IR section
-Enable IR blaster (I have the HD-PVR from Hauppage).
-Start MythTv > Remote does not work
-Exit MythTv > remove IR blaster selection > start MythTv > Remote works
-Repeat last four steps...very repeatable.

-Copy these lirc files from old and working MythTv box (12.04) to this
new installation
-Note: saved off the files from the installation
   -key definitions
   /home/don/.lirc/mythtv

   -include list for applications
   /home/don/.lircrc

   -driver configuration
   /etc/lirc/hardware.conf

   -includes for blaster setups
   /etc/lirc/lircd.conf

-Start MythTv > Remote works, IR blaster works
-Restore each of the above files one by one and try MythTv > works every
time

It seems the act of using the old files fixed something...not sure
what. I left the 12.04 lirc files in place and continued with other stuff.
