I've had it happen two or three times.  At least one of those was with 
"name-brand" RAM (Kingston), so I don't think quality was a factor.

I've gotten generic RAM that was bad out of the box on one or two 
occasions.  I test *all* RAM after installing it as a matter of course. 
  Because of this, and because it can be so hard to be sure that I'm 
ordering the right kind of RAM for a particular system, I try to only 
buy RAM from vendors with good RMA policies.
