Hmmm...Not sure where you are, Greg.  I recently switched over to Time
Warner Business Class internet service (I'm 40mi outside of Los Angeles). 
They give me five usable static IPs (/29 block), 15Mb/s down and 2Mb/s up,
and as mentioned above, no blocked ports or restrictions on servers (I run
four facing the 'net).  $119/month.  Not cheap, but only twice as much as
my previous pokey DSL service cost, with 4-5x the speed and better
reliability.  As was mentioned previously, their customer service is MUCH
better than the residential side.

My guess is that the further you are from a big city the more the prices
go up and the choices down.

As far as TV goes, unfortunately cablecos are essentially granted
monopolies here (I have TWC for my TV service as well, and if I wanted to
switch to another carrier I'd have to go with AT&T Uverse, which isn't
really "cable" and wouldn't work with my Myth machine's tuners; OTA is not
a reasonable option...that 40mi between me and the broadcast towers
includes a couple of mountain chains).
