[mythtv-users] PVR-250 recording only channel 4?

I had this same issue. I wish I would have kept better notes on it... I 
think what I ended up doing was removing and re-adding the ivtv rpm's, 
and rebooting a few times. Check the permissions in /dev/ I think that 
was what my problem really was.
