have you used the mythfilldatabase --manual option?

what did you use to generate the xml file?
have you run myth's setup? this will setup the cardinput table, which is 
needed by mythfilldatabase when you use --file.

mythfilldatabase --file <sourceid> <offset> <xmlfile>
    sourceid = comes from the cardinput table
    offset = days from today that xmlfile defines
             (-1 means to replace all data, up to 10 days)
    xmlfile = the actual program data
