[mythtv] HDTV patches

One more try, I found another potential bug that could only effect xvmc
with MV acceleration.

If this doesn't work I'll have to try to get my hands on a GeForce 4Ti card.

]Doesn't fix the problem here.  Still makes the video look like crap.  Very
]grainy, big blocks of uninitialized video.  I frames are clear, though.  On a
]positive note, though, it no longer crashes on one of my test clips, so,
]there's some improvement with this updated patch.
