[mythtv] [PATCH] Cleaning up card input data members

Isaac,

Following up on the card-input sourceid additions, I went ahead and 
created a CardInput class.  No functional changes, just purely 
structural re-organization. 

I hope to streamline some of the clients of the class in the near 
future.  Additionally, I'm toying with a way to say tune to channel "x," 
and having this class play a role in choosing the input, channel, tuner, 
etc.  That may or may not work out.  But I'm trying to creep to a place 
where the HD-2000 card can exist in Myth as a dual mode ATSC/NTSC tuner, 
sans hacks.

I've attached the patch and the two new class source files (.h and .cpp)
