Few things to try: 



    1. Are you running kernel 4.3.x? Even without those modules loaded I've seen this same issue, try 4.2 instead 
    2. Try a different, non-USB3, port 
    3. The power adapters on these things are notoriously bad. I had one go bad on me and the symptoms were the same as what your seeing. I have a box of old AC adapters I've saved from old devices and happened to have a replacement 5 v @ 2a. That solved my issues. If you don't have a spare Hauppauge sells replacements but anything with the same specs will do.

I connected the HD-PVR to a new box and had similar issues. Kill-a-watt was showing 9 volts and .04 amps under no load. I did not try it when it was under load. 

I then purchased a new power supply. After five hours or so of varying length recordings, it has not had any problems. One thing I notice the blue light seems brighter. 

In the final days I was seeing disconnects and reconnects. It would even go from /dev/video0 to /dev/video1
