After all that hassle I think this just boils down to a problem with the 
remote configuration file I was using. I used irrecord to start a 
partial config file and viola! irw produces output:

Now the configuration I have been using for *years* is a Scientific 
Atlanta 3250. It had an intersection with an inexpensive, multi-device 
remote that included the four A, B, C, D buttons I found myself relying on.

Now there *is* a configuration in the database for this remote but it 
doesn't appear to work with LIRC 0.9.0. Seems like everything in the 
database has been generated well before 0.8.6.

If anyone can suggest any other A-D remotes other than the MCE remote 
which is never supported by programmable remotes, let me know.
