I had that very same card and it did the same thing to me.
Using nvidia-smi I saw really high temps.  When it got really hot the
picture would freeze and other bad things happened.

The fan on the card WAS WORKING.  I physically saw it spinning but when I
actually checked close, it was dirty and hard to spin by hand.
So I replaced the fan with a bigger fan that I rigged to the heat sinks.
Ever since then, around 2 years, it has operated well within specs with no
issues.

You might try replacing the fan with something better.  It certainly will
not make it worse.
