I recently tried to turn on "automatically lookup metadata" option on all my recording profiles but I noticed that they are still erring out with: Failed with exit status 128. Does anyone know how to fix this or even debug it? I tried removing the config.xml again and it didn't fix the issue. I've looked at my mythpreviewgen logs and I don't see the orphan problem. Any ideas?

Do you have a zero byte config.xml file again? If it keeps getting over-written maybe try getting the file correct then removing all write permissions, ie chmod 444 or 440 or something similar. Removing it may not be sufficient if it fails to be regenerated correctly due to whatever the bug is.

I updated myth last night and this morning I started seeing mythpreviewgen error 128 again on two recordings from last night. I started digging into it and I found that my config.xml file went to 0 bytes again! :( Is there something I can do in order to keep this file from getting zeroed out again?
