Have you tried cold-booting ?

A couple of weeks ago I belatedly noticed one of my tuners - a Kworld/Peak  
PCI clone - was consistently failing with similar errors on warm starts. I  
don't remember if I'd updated the kernel before that.

However after a cold power cycle it rectified itself.

I also have a UB499-2T that was unaffected - but that's on a powerless USB  
socket so it always cold-starts.
