I've tried several iterations.  Uncompressed audio, removing the high
quality flags, single threaded vs multi-threaded, MP3 audo vs.
uncompressed audio etc.  Each fails at roughly the same place (roughly
6% into the video).

Then I tried a different video.  Bingo.  Bad video file.

Sigh..  I need more coffee.
