I disagree with your logic I'm afraid, if I have devices inside the
case I can have cooling and fans to control Thermals, not so easy with
a USB device outside the case.  I've used a Hauppauge USB Dual Tuner
stick in the past that was really quite hot to touch when under
extended load, hotter then I personally would find acceptable for a
device 'hanging loose' as it were.  I appreciate your comment about
slots being an issue but I use Asus 6 and 7 slot PCI-e motherboards to
solve that problem, swapout for any of my cards can be done in under 2
minutes using thumb screw cases.
