The cache purge caused by switching themes purges /everything/. The
"Pre-scaling Theme Images" that occurs on application startup only
re-scales those /theme/ images that have changed since the cached image
was created (i.e. where the filesystem timestamp on the cached image <
the timestamp on the theme image).
As for cached metadata, I'm sure it works like the theme pre-scaling,
but occurs during usage (when the specific scaling/image operations are
known).
