The theme cache can only be cleared entirely, there's no partial cache
clearing.  So yeah, sometimes changes to the theme won't be evident to
the user until the cache for that theme has be cleared.  This only
pertains to changes w/ graphics files/effects.  So as an example, if I
remove the reflection effect from MythVideo Gallery view, all your
covers that are already cached will stay reflected until you clear the
cache.
