-- The MFG. expected life of an antenna is about 10-15 years.  Of course they want to sell you a new antenna!  YMMV  Good cable may cost more than a new antenna.

-- You should use RG-6 Quad Shield for shorter runs or consider RG 11 if the length is over ~100 feet.  You will loose more signal at the higher channels first.  You may need a Mast head pre-amplifier, these are 'special' low-noise amplifiers intended to account for losses in the cable to the house, and are different than distribution amplifiers.

The NEC is pretty specific about how to ground a system.  So, Yes it needs to be grounded, and sorry I can't explain with proper detail...  The antenna and the coax are grounded.
