Does it have to be a card?  I really like my HDHomeRun from Silicon Dust.  Two digital tuners built into a small box, talks to any machine on your network via 100 Mbps Ethernet.  Handles ATSC OTA or QAM.  My antenna and cable feeds just happen to enter the house next to my networking gear.  So that's where my HDHR lives.
