How many streams are you trying to write at the same time. I used to
have a problem similar to yours if I tried to record two programs at
the same time, or if anything else was accessing the disk in addition
to the recorded program.
I solved that problem by re-configuring the PCI latency values of the
disk subsystem, now the same system can record 4 programs and run a
commflag job at the same time without problems.
You mentioned that it only happened with some programs, could it be
just when you are recording more than one? Or perhaps running
Mythburn or something similar at the same time ?
