What user are you running irw as ? Does that user have permission for  
the lirc devices.

Is lircd actually starting and continuing to run, check it with 'ps  
aux | grep lircd' a few minutes after it is started to make sure it's  
still running.

Is lircd perhaps starting before the devices are created somehow?

Try running ldd on lircd to make sure it is finding all if its  
required libraries.

Just some random wild ideas, but it does seem as though you are  
getting closer. You really do seem to have checked all of the obvious  
stuff. I'm glad it turned out to not be a hardware problem.
