So set VertRefresh to 50-50, as you set 60-60 in your example?

What's the effect of running a refresh rate in X that doesn't match the
TV rate? Does the card really output the specified refresh rate, or is
it always 50 Hz out for PAL, for example?

To be honest I can't tell what I'm getting out of X. I have defined a
800x600x50 mode, but I'm not using it yet. The X log tells me what valid
modes exist:

(**) NVIDIA(0): Validated modes for display device TV-0:
(**) NVIDIA(0):      Default mode "800x600": 50.0 MHz, 48.1 kHz, 72.2 Hz
(**) NVIDIA(0):      Default mode "640x480": 25.2 MHz, 31.5 kHz, 60.0 Hz
(**) NVIDIA(0):      Mode "800x600x50": 33.2 MHz, 31.4 kHz, 50.0 Hz
(**) NVIDIA(0):      Default mode "800x600": 40.0 MHz, 37.9 kHz, 60.3 Hz
(**) NVIDIA(0):      Default mode "800x600": 36.0 MHz, 35.2 kHz, 56.2 Hz
(**) NVIDIA(0):      Default mode "400x300": 25.0 MHz, 48.1 kHz, 72.2 Hz (D)
(**) NVIDIA(0):      Default mode "400x300": 20.0 MHz, 37.9 kHz, 60.3 Hz (D)

but doesn't say which one it is actually using, only:

(II) NVIDIA(0): Setting mode "800x600"

which could be any of 3 different refresh rates.

Is the correct refresh rate (ie 50 Hz for me, 60 for you) required for
"experimental A/V sync" to work and be useful?
