Can you let me know which X driver and modeline you are using for 50Hz? Also, 
if you are using the 'nvidia' driver, you might try enabling Experimental A/V 
sync in Myth, that helps too.

The mythtv settings (I believe) all relate to capture, not playback. For 
playback you set this by using the "DisplaySize" parameter in XF86Config - 
search the archives for more detailed examples, I don't have one handy right now.
