Jeremy,
One thing that I noticed with fbset -x is that even
though your display is okay, XFree86 will override any
modelines that it thinks might blow up your
monitor/TV.
Try checking /var/log/XFree86.0.log, this should give
you some idea why your mode was rejected. You'll then
need to adjust it (either vertically or horizontally)
until it fits.
If it's the vertical, you might be able to get rid of
the "VertRefresh 60" line, since you're close enough
to 60 that you probably won't blow up your TV. :)
