You might check and see if there is contention going on over the sound
device, specifically /dev/dsp1.  I had occasional slowdowns exactly as
you described that were caused by btaudio and my PVR-250 both fighting
over /dev/dsp1.  Adding additional options to btaudio telling it to use
/dev/dsp2 instead of /dev/dsp1 cleared up the problem.

Of course, your problem probably isn't btaudio, but it could be
something else, such as the aRts soundserver, etc.
