Update: I got it working with mplayer and the
following options, I now get DD5.1 through my S/PDIF. 

mplayer dvd://35 -dvd-device /dev/dvd -fs -zoom -vo xv
-ac hwac3 -ao alsa9

However, MythTV does NOT output anything through the
S/PDIF output (not even stereo) with the same
alsa-mixer settings.
