mythbackend is like a daemon that runs... you could think of mythbackend as
a server and mythfrontend as a client.  the backend does everything related
to controlling the TV controller hardware, database access, etc.  the
frontend accesses the backend to get its data (channel info, video streams,
etc) and the frontend is the program that runs the GUI that the user sees.

I think for the most part, most people see the exact same thing on their TV
as they would on their computer monitor. There are video cards that let you
seperate the outputs more, so only the myth would be displayed to the second
output, but I'm not sure that many have/use/care about them?
