[mythtv-users] Playback speeds/slows after a few minutes

This sounds to me a little like your clock rates in your sound card being out of rate with your mpeg rate.  If it's a little slow then the buffer is starting to empty (or overfill).  

Try the option to keep extra data in the audio buffers?  Try a different sound card?
