[mythtv] [PATCH] Bootstrap mysql.txt in mythcontext

GUI bootstrap for frontend
(and anything else that creates a mythcontext with gui==true)

If there is no mysql.txt file loaded, put up a simple
(non-themed) dialog to get the backend settings,
and then write them into $HOME/.mythtv/mysql.txt


cvs diff -u libs/libmyth/mythcontext.cpp >../patch.general.2
