[mythtv] CA & charset patch (for experimental DVB patch only!)

Here's my patch for Conditional Access with the new DVB code (v1.1). 
Also included is cleaner and better charset handling for 8-bit character 
codings (no multibyte stuff yet).

It works for me but this should be considered beta code so all the usual 
warnings apply: If you use this, things might explode, your cat might 
die, you might spill your coffee and so on. However, if you're brave 
enough to be testing the experimental DVB code this should probably be 
safe too. Please report failure/success.

If the CAM is working you should see something like this in the log:

Myth users who live in ASCII land and don't use CA probably won't notice 
any changes with this patch but you are more than welcome to try it 
anyway, to make sure I haven't broken anything.

Taylor, could you please merge this with your code and include it in the 
next release of your patch? Thanks! =)
