[mythtv] [Experimental PATCH] New FF/Rew mode

This patch reworks the fast-forward/rewind mode to hopefully be
smoother and provide more accurate timing.  I think it's close enough
for me to commit but I'd like to get some feedback before doing so.

Here is the list of known issues.  None of them are serious.  Please
let me know if you find any others.

  When using ivtv output on live TV or an active recording,
  fast-forward mode doesn't always stop automatically when it gets
  close to real-time.

  When changing speeds, there is a noticable pause before the new
  speed takes effect.

  The automatic repositioning factors are all cleared because the
  implementation is completely new.  Please make suggestions.
