[mythtv] [PATCH] Control what info get from LCD display, plus a timeformat improvement

Hello,
attached there is the patch I submit last week adjusted as suggested by
Isaac, moving all check in the constructor.

Patch adds an LCD device screen on the appearance settings menu. Through
this screen the user can choose what info see on the LCD display. For
example I can deselect the menu or the time lcd screen.

Note: to minimize database access hammering, every lcd screen is checked
only one time when mythfrontend starts. So, if you change the any lcd screen
appearance, you have to restart mythfrontend.

For the same reasons, the timeformat check for time lcd screen format is
done only when mythfrontend starts.
