[mythtv] MythTV 0.15 Port for FreeBSD

OK, so at this point, the remaining patches are:

patch-channel.cpp           patch-playbackbox.cpp  patch-videoout_ivtv.cpp
patch-mainserver.cpp        patch-tv_play.cpp      patch-vsync.c
patch-mythcdrom.cpp         patch-tv_rec.cpp
patch-mythmediamonitor.cpp  patch-videodev_myth.h

The rest are merged and/or no longer necessary.

- Several of these have to do with setting the thread stack size; it'd be nice
  if we could fix that another way instead.

- I haven't merged that clamp() change yet; I'd like to know what's going on
  there first; maybe it's a g++ problem

- The ioctl changes for the CD-ROM etc. should probably result in some new
  functions so this can be done cleanly

- Regarding patch-videoout_ivtv.cpp, isn't roundf(3) part of C99?  Does it
  really not exist on FreeBSD?
