[mythtv] [PATCH] ALTPCM/PCM2 support

For some sound cards you need to be able to use ALTPCM/PCM2 to control sound
(i.e. sblive with SPDIF out), This just adds that option to the general
configuration. The PCM slider initial volume level is used in case you
select PCM2.
