[mythtv] [PATCH] DVB Fixes

Hi,

I know I said that I wanted for Augusto to resubmit the patch,
but I was doing going to submit the changes below myself,
so I cleaned up Augustos patches too.

(Please, use this list in commit message, and not just dvb fixes,
 as that makes it quite hard to track changes)

Augusto Cardoso:
	- Swapped source/header in .pro
	- Move static variables to ipack struct
	- Generate correct data rate for mpeg remux
	- Fix recordts option.
	- Fix a segfault in dvbcam
	- Improved/fixed continuity counter error detection
	- Set driver buffer size for pids
	- Correct the dvb_sat table (typo in prev update)

Me:
	- Move dvbsection/dvbcam to dvbchannel for faster
	  channel change/startup.
	- Remove keyframedist/gopset (Unused)
	- Only set available CAMs

Could you fix the dos format and exec flag on videoout_directfb?
