I'll second picoPSUs. They simply plug on top of the power connector on the motherboard so take up 
very little space in the enclosure. They are also quite happy if you don't have any hard disk in the 
box; I had problems in the past using a flex-PSU which wouldn't work unless there was a sufficient 
12v load, say from a disk drive.

The input - at least on the two I have - is a 12v power connector which goes on the back panel 
somewhere. You can drive this with a power brick, with the losses others have mentioned, or from a 
battery bank or even a 12v output from another PC.
