Losing channels is sometimes a result of poor OOB
lock (channels are marked as subscribed by receiving
entitlements via the OOB path).  While the OOB data
is repeated again and again, and is tolerant of poor
quality, it does require 'adequate" signal.  What does
your OOB numbers show?

Poor OOB is (usually) an issue with verizon equipment
for fios, or bad cabling (or connectors), or interference
from something nearby, but as verizon is in the process
of converting to IPTV delivery in some (test) markets,
there may be more than meets the eye here.

In any case, if the OOB looks bad, the first thing to do
is connect the prime with a (few) meter high quality
coax directly to the ONT and see if the numbers get
better.  And (of course) make sure the CableCARD
shows as validated.
