I tried this myself, If I set it to always start live TV in the guide,
it crashes when I exit Live TV. Also, as you saw, the guide page is
blank with the preview video showing in the corner.
I checked and found that this option is no longer available in 0.28. The
recommendation is to select Program Guide from the main menu, scroll to
what ever you want to watch, press M or Menu, and select "Watch this
Channel". The guide page is not blank, and it works every time I use it.
The error does not happen on 0.27 without "always start live TV in the
guide" set or on 0.28 , even when going into the guide from within live
tv and selecting another channel, then exiting.
My recommendation is turn off "always start live TV in the guide", you
will not have it anyway in 0.28. I am not going to spend time looking
into this since it is not an issue in 0.28.

I still am not able to recreate your offset video. Please let me know if
you have "Separate video modes ..." Checked on Setup -> Appearance
second page. If so please unselect it. I did manage to create a
distorted video by putting wrong values in there, but not quite how you
are seeing it.
Please can you also send me a copy of your /boot/config.txt
What is your screen resolution?
In the log it displays the screen resolution, like this:

What do you see in your log? Does it match what you expect?
