I'm not an expert at this either so hopefully someone with more knowledge
will chime in. It looks like commercial flagging is done as the program is
recorded and on completion the recording is transcoded. I don't think this
alone will remove the commercials from the recording. Commercial flagging
allows a Mythfrontend to "skip" commercials during playback. To remove the
flagged parts of the recording you need to "copy" them to a cutlist and
then transcode the recording. IMHO, the best way to do this is to start
playback on the frontend and open the editor (hit "e") and bring up the
edit menu (hit "m") then you should see an option to "load" the cutlist (I
don't use automatic commflagging so I don't know what the exact option is).
At this point I would verify that the cutpoints are actually where you want
them (moving them as necessary) then exit the editor (hit "e"). Now you can
start the transcode job and the marked segments will be removed.

Don't know where you are located but where I am in the US automatic
commflagging doesn't work well at all (not MythTV's fault). It is so bad,
in fact, that I don't use it at all, as I mentioned above. I build my
cutlists by using the editor to jumping through the recording and mark the
parts I want to remove manually.

If you trust the automatic commflagging you can use mythcommflag to create
the cutlist, see examples of this on the wiki like:
