And "not found in PAT" suggests that the stream is not being
received.

During the time of the failed recording, you will need to use
the ceton web interface and determine why.

By any chance does your franchise use TAs?  If it only
happens "sometimes", there are known issues with some
franchises, and some TAs, that fail to tune as reliably (or
as quickly) as most OCUR devices expect.  Again, the
details should (eventually) be findable in your Ceton
card.
