There were some odd omissions from the list, but the HD programs
available OTA, including 801-811, were enabled.

As I wrote above, I use Schedules Direct, and the OTA HD channels were
enabled. It turns out their linup did not include 40=MSNBC or 107=AJAM
(I submitted a correction).
Anyway, I enabled all of the programs I subscribe to, and deleted the
old lineup showing analog channels, and re-ran
mythfilldatabase --dd-grab-all
but MythTV still crashed when I tried to view WHDH HD :-(

I don't see "pairing" mentioned in any of the HDHR web pages. Where should I
find that?
I'm not much concerned about the empty "subscribed channels" listing
since the channels are listed as "subscribed" when I tune to them, and I
can watch them with VLC.
In fact, since this chain works with both SD and HD encrypted programs:
HDHR - CableCARD - Ethernet - NIC - VLC
and this chain works for encrypted SD programs:
HDHR - CableCARD - Ethernet - NIC - MythTV
I am inclined to think the reason the second chain fails with HD
programs has something to do with MythTV, and not the CableCARD.
Eric Sharkey <eric at lisaneric.org> makes this point:

So what should I check about the setup for those HD programs? For that
matter, how do I dump the database so I can conveniently display the
