Fixed

I decided to try to flip everything over to Pulse.  The FE setup is simple,
just scan and pick.  The backend referred me to the pulse audio source.
Not so useful.  I did find a hint in the Ubuntu bug logs -
https://bugs.launchpad.net/ubuntu/+source/mythtv/+bug/633452

The fix is to add PULSE_INTERNAL=1 to the exec /usr/bin/mythbackend line in
/etc/init/mythtv-backend.conf

I have it running with a SoundBlaster X-Fi USB for now.  I'll put the other
card back in later.
