I believe the "life happened" to the particular dev who was
the most interested/active in improving this area of the code(*).
Some minor code changes have happened, but AFAIK
nothing substantial has been committed to master.

Given the large variety of ways commercials are added
to the content around the world, it is certainly not an easy
task to get detection to work well for all locations.

Personally, since before time (i.e. an old old old (and now
dead) VCR), I have tended to just use the 30 second
skip function. While correct flagging would be better,
I have not (yet) lost the ability to push the skip button.

(*) "Life happening" has impacted the plans for other
intended and desirable features too. But one has to
be happy for the devs whose "life happening" include
things like new opportunities (professional (new jobs)
and personal (marriage)). The best solution is for others,
if they have the capabilities, to contribute with code
improvements. Commflagging might be a bridge too
far for (almost) anyone, but last I looked there were
other suggestions at http://code.mythtv.org/trac
(Wanting to help out?) to look at (should any of
them also be an itch you are interested in). I will
point out that fixing some existing open tickets would
release developer time to work on the harder things.
