It may (or may not) be related, but the pvrusb2 driver (maintained
by Mike Isley) has exactly this type of race condition during
initialization for a HVR-1950 (which has a digital and analog
side, although I seem to recall somewhat different chipsets
under the covers).  I sent him a (mostly reliable) reproducer
at one point in the past, and he indicated he knew the area
that needed to be fixed, but it was going to take some time
to look at.  In my cases, about 5% of the time, the device
failed to initialize correctly at boot/hotplug time.  I have since
eliminated my use of those devices, so the issue is moot for
me.  It is possible that touching bases with Mike might jog
his or your memory regarding the details of the various race
issues.  Or it may be a wild goose chase.
