He specifically mentioned at the end of his article :

Now, after making these changes, I got sound over HDMI in MythTV if I 
specified “ALSA:HDMI” (or “plughw:0,3”) as the output, but any Flash 
audio (e.g. MythWeb or HuluDesktop) and system sounds did not play since 
I couldn’t specify the output. The problem turned out to be a 
~/.asoundrc configuration file that was overriding my /etc/asound.conf 
file. Removing ~/.asoundrc forced the /etc/asound.conf file to load and 
make HDMI the default ALSA output. Only then could I switch the MythTV 
audio to “ALSA:default” and Flash-based audio and all system sounds 
started to work.

The key I think is that you are specifically telling MythTV to use the 
HDMI, you need to do the same in the OS.

On my systems (OpenSuSE 12.1), that entails disabling PulseAudio, and 
configuring phonon through the Kmix app to use the HDMI output.
